BASIC LIFE & AD&D INSURANCE
Life Insurance & AD&D provides additional financial protection for you and your family in the event of your death. WholesomeCo provides you with $25,000 protection. This benefit is provided at no cost to you, through Kansas City Life.

​VOLUNTARY ACCIDENT INSURANCE
Accident insurance, through Kansas City Life, can help with medical or other costs associated with a covered accident or injury that your health insurance may not cover.
VOLUNTARY LIFE INSURANCE
Voluntary Life Insurance is offered by Kansas City Life. This benefit will enhance the limit provided to you by WholesomeCo. You may choose coverage at a minimum of $10,000 and a maximum of $500,000 or 5x (times) base annual salary (whichever is the lesser). Amounts in excess of the guaranteed issue amount of $100,000 will require evidence of insurability. Coverage for employees is offered in increments of $10,000.
Coverage for spouses is offered in increments of $5,000. Amounts in excess of the guaranteed issue amount of $25,000 will require
evidence of insurability.
Coverage for children is offered in increments of $2,500. Coverage for your spouse and children cannot exceed 50% of your election. Infants will be covered for $1,500. Infants are defined as being fourteen days of age to six months, and children are six months of age to 19 (or 25 if a full-time student).

If you elect new life insurance or increase your current coverage more than $10,000 at open enrollment, please complete the Evidence of Insurability form and mail it to the address located on the form.
